Ambroxol is a metabolite of bromhexin having mucolytic and mucokinetic action and capable of inducing thin copious bronchial secretion. It depolymerises mucopolysaccharides directly as well as liberating lysosomal enzymes that breaks up the network of fibers in tenacious sputum. It stimulates ciliary activity thereby improving mucokinesis (transport of mucous). When administered orally, onset of action occurs after about 30 minutes.

Guaifenesin is thought to exert its pharmacological action by stimulating receptors in the gastric mucosa. This increases the output from secretory glands of the gastrointestinal system and reflexively increases the flow of fluids from glands lining the respiratory tract. The result is an increase in volume and decrease in viscosity of bronchial secretions. Guaifenesin produces its expectorant action within 24 hours.

Levo Sulbutamol is a highly selective β2 agonist .having action of bronchodialation . Activation of beta2-adrenergic receptors on airway smooth muscle leads to relaxes the smooth muscles of all airways, from the trachea to the terminal bronchioles. Increased cyclic AMP concentrations are also associated with the inhibition of the release of mediators from mast cells in the airways. Levosalbutamol acts as a functional agonist that relaxes the airway irrespective of the spasmogen involved, thereby protecting against all bronchoconstrictor challenges.

Indication: Bronchitis, Bronchial Asthma, Wheezing, Productive Cough, COPD, Fever, LRTI.
Drug Composition: Each 5 ml of Pulmorest LS contains, Ambroxol 30 mg, guaiaphenesin 50 mg and Levosulbutamol 1 mg.
Dosage: Adults - 1 tsf 3 - 4 times / day. Children under 6 years - 1 tbsp b.i.d. Children 6 to 12 years - 1-2 tbsp t.i.d.
Contraindications: Hypersensitivity, Eclamsia, IHD.
Precautions: CVD, DM, Glucoma, Hyperthyroidism, Tremor, Palpitation, Tachycardia, Pulmonary edema. Alcohol causes lactic acidosis, β blocker results in serious bronchoconstriction. Theophylline or Aminophylline leads to hypokalemia.
